Hospice Care at Home in Worcester County & MetroWest

Comfort, dignity and support for your whole family. Preferred Peaceful Transitions Hospice brings expert end-of-life care to your home, fully covered by the Medicare Hospice Benefit.

When the goal becomes comfort, we are here.

Deciding on hospice is one of the hardest choices a family makes. Hospice is not giving up. It shifts the focus from treating an illness to easing pain, managing symptoms and making the most of the time you have together, at home with the people you love.

Our hospice team carries the same “Love Your Patient” philosophy that made Preferred Transitions one of the top-rated home health agencies in Massachusetts. We take time with each patient, listen to what matters most and support the family every step of the way.

Who can benefit from hospice care?

Hospice is for anyone with a serious illness whose doctor expects a life expectancy of six months or less if the illness runs its usual course. Families often call us for loved ones living with:

Many families tell us they wish they had called sooner. Starting hospice earlier gives patients and caregivers more months of support, not fewer.

Your Hospice Care Team

Patients, families and caregivers are key contributors to the plan of care. Your team builds that plan around your wishes, works closely with your own doctor and brings in speech, physical and occupational therapists when needed.

Medical Director

Oversees your care plan and works with your primary care doctor on pain and symptom control.

Nurse Case Managers

Visit regularly to manage pain, breathing, anxiety and other symptoms, and teach family members what to expect. Our on-call team is available after hours, on weekends and on holidays, 24/7/365.

Hospice Aides

Help with bathing, grooming and personal care so your loved one stays clean and comfortable.

Medical Social Workers

Caring Volunteers

Provide companionship and give caregivers a break.

Grief & Bereavement Support

Medications, Equipment & Supplies

Respite Care

Arranges a short stay in a facility, up to five days at a time, so family caregivers can rest.

Hospice Care Is Wherever You Call Home

Hospice is a philosophy of care, not a place of care. We provide care wherever you call home: your own house, an assisted living community, a group home, a hospital or a skilled nursing facility. When pain or symptoms need more support, additional levels of care are available.

Hospice is covered by the Medicare Hospice Benefit.

Medicare Part A covers hospice care related to your terminal illness. Most families pay nothing out of pocket. There may be a copay of up to $5 per prescription for comfort medications and a small coinsurance for respite care. You qualify when:

  • You have Medicare Part A
  • Your doctor and the hospice medical director certify a life expectancy of six months or less if the illness runs its normal course
  • You choose comfort care instead of treatment aimed at curing the terminal illness

Medicare still covers care for health problems unrelated to the terminal illness. You can stop hospice at any time and return to regular Medicare coverage. Hospice is also covered by MassHealth (Medicaid) and other private insurance.
