Exceptional home health care that treats you like family.

Coming home from the hospital, recovering from surgery or managing a new diagnosis can feel overwhelming. Our clinicians come to you, assess your progress, teach you and your family what to watch for, and keep your doctor updated.

We built Preferred Transitions in Milford in 2016 around one motto: Love Your Patient. Our clinicians see fewer patients per day than the industry average, so your visit is never rushed. That approach is why Newsweek named us one of America’s Best Home Health Agencies for 2026 and why Medicare data places us in the top 2% of agencies in Massachusetts.

Home health services we bring to you.

Skilled Nursing

Registered and licensed practical nurses manage medications, check vital signs, monitor chronic conditions like heart failure, COPD and diabetes, and catch warning signs early so you can avoid a trip back to the hospital.

Physical Therapy

Physical therapists and PT assistants help you get stronger, walk safely and get in and out of bed, the car or the shower. They also check your home for fall risks.

Occupational Therapy

Occupational therapists help you get back to everyday tasks like dressing, bathing and cooking, and recommend simple equipment that makes home safer. [H3] Speech-language pathol

Is home health care right for you or your loved one?

Home health care is a good fit if you or a family member:

  • Just came home from a hospital, rehab or nursing facility stay
  • Had surgery, such as a hip or knee replacement or heart surgery
  • Has heart failure, COPD, diabetes or another condition that needs close monitoring
  • Has had a fall or is at risk of falling
  • Are recovering from a stroke
  • Has a wound that needs regular professional care
  • Has new medications or equipment and needs help learning them
  • Has a change in medical condition

Home health care is covered by Medicare.

If you have Medicare Part A or Part B, you pay $0 for covered home health visits. You qualify when:

  • A doctor or other approved provider orders home health care and oversees your plan of care
  • You need part-time skilled nursing or therapy
  • You are homebound, meaning leaving home takes considerable effort, such as needing a cane, walker, wheelchair or another person’s help
  • You can still be homebound and go to medical appointments, religious services or adult day care.
