Preferred Transitions was founded in 2016 by John Fletcher and Dominic Butera.
John Fletcher is a nurse with over 25 years’ experience. He provided exemplary care to patients in the hospital setting and several skilled nursing facilities as well as developing close ties in the medical community during that time. He wanted to start his own homecare agency that would not only provide excellent patient care but also evoke the qualities of “old fashioned visiting nurse associations” ie a true community based organization that would provide compassionate care with patient centered goals and satisfaction at its core.
Dominic Butera was a retired entrepreneur in the manufacturing sector that was inspired through discussions with John to help people in need. This was a new venture and Dominic brought passion and enthusiasm to the idea of creating a medical model homecare agency that would set them apart from existing agencies. With research and outreach to local institutions the foundation for Preferred Transitions was created with the following priorities-
- 1st Patient Care, Satisfaction, Outcomes
- 2nd Clinicians and Office Staff Satisfaction with a good work/life balance
- 3rd Agency Development as growth dictates
Their intent was to create a homecare agency where patient centered goals were a priority as well as providing a work culture for clinicians that puts patient care first. Our clinicians are compensated higher than other agencies and have lower daily productivity than the average homecare clinician thereby allowing for more time spent in the patient’s home.
We at Preferred Transitions have made it our mission to emphasize patient care above all else. In Dominic’s words this means “Loving your patient” which translates to giving excellent, holistic clinical care. The kind of care we would want for our loved ones defined as-
- Providing care that results in quality outcomes for the patient
- Providing care that is individualized to the specific patient needs
We accept nothing less.
Preferred Transitions accepted our first patient in July 2016 and cared for 65 patients during our first year. In 2017, we grew to provide care to over 400 patients and in 2018 we exceeded 800 patients. We have been growing ever since and believe the reason for this growth is our philosophy for patient care and staff satisfaction. This is illustrated by our continued Medicare Patient Satisfaction Survey Rating of 4-5 Stars.
