1) Director of Nursing Job Description and Duties
· The Director of Nursing is responsible for coordination of care and is a liaison for patient and clinician oversight.
· Available for oversight during all working hours of the agency.
· The Director of Nursing will assist with committees, development of policies and guidelines for and obtaining input from clinical personnel and providing professional input to related administrative systems.
· The Director of Nursing will assist with chart audits, field clinician supervisory visits, orientation, education, overflow of patient visits in the field if needed and triage of field clinician questions.
· Assists with coordination of orientation and in-service programs.
· The Director of Nursing interprets service needs to the Administrator/Chief Clinical Officer and acts as a patient services administrative liaison.
· The Director of Nursing will perform nursing visits in the field and annual performance evaluations in the field as needed.
· The Director of Nursing, in accordance with policies and standards established by the Governing Body, and under the direct supervision of the Chief Clinical Officer, assists with the planning, coordination, leading, controlling and evaluation of home health services.
· Consults plans and works with Administrator in policy formulation and programmatic development and implementation including assessing adequate and appropriate staffing.
· Helps to develop standards that ensure safe and effective care for patients and families.
· Assists with maintaining policy and procedure manuals and updates procedures and job descriptions as policies and procedures change.
· Assesses appropriate staff supervision during all service hours.
· Assists in marketing the Agency and fostering community relations.
· Consults with physicians in matters relating to patient care services.
· Coordinates patient care.
· Coordinates referrals.
· Ensures that patient needs are continually assessed.
· Triages patients, families, physician, and staff telephone calls.
· Consults with supervisors and staff, individually or in groups, regarding patients or special programs or service programs.
· Oversight of Nursing Supervisors and nursing teams.
· Oversight of scheduling.
· Works closely with the Rehab Director to provide guidance to all staff in the field and office.
Oversight:
2-3 Nursing Supervisors, 2-3 Schedulers, Nursing Team 1, Nursing Team 2, Weekend Nursing Supervisor, 2 Field Nursing Supervisors Weekday, Home Health Aides, and Social Workers
At least 4 days in the office weekly (1 day remote available after fully trained (6 months))

2) Nursing Supervisor Duties and Job Description
· Supervise Nursing, Social Work and home health aide staff
· McKesson Approval
· Payroll
· Relias oversight/HR oversight
· Perform duties as a home health nurse in the field – only if staffing crisis
· Running reports to ensure we are meeting regulations:
o Staff visit note completion
o OASIS visit note completion
o HHA supervisory report (?? Custom reports)
o LPN supervisory report (?? Custom Reports)
· Oversight and assist with the scheduler as a team regarding the nursing, social work and home health aide schedules.
o Manage sick calls and call outs
o Manage PTO requests and staffing based on them
· Joint visits with your direct report field staff
o Annual joint visit and evals with your clinicians
o Annual joint visit and competencies with your clinicians
· Oversight of IDG meetings weekly
· Performing Case Conference with all your clinicians weekly
· Triaging calls from clinicians in the field
